25 | Licensing Program Analyst (LPA) Dean Thompson conducted a case management investigation to follow up on a self-report unusual incident that was submitted to CCL Regional Licensing Office on 12/13/2024. Upon arrival LPA met with Director Ashley Neitzke then toured the facility inside and outside. Census was taken, there were 49 preschool-age children with eight staff.
During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 12/13/2024, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported on 12/12/2024, child#1 (C1) is allergic to egg and C1 was given a food item that had an egg ingredient. C1 prescribed medication (Zofran) was administered by staff #1 (S1) approximately 10 minutes after eating the food item. S1 stated C1 was fine after the incident and didn’t show any signs of an allergic reaction. Since the incident, Director stated any food that leaves the kitchen must be reassessed for any allergies.
Based on interviews which were conducted, and records reviewed, the facility was issued a Technical Violation during today's inspection.
Exit interview conducted and report was reviewed with Director Ashley Neitzke. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Appeal Rights discussed. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. |
