On 07/15/2024 at 12:00pm, an unannounced case management inspection was conducted by Licensing Program Analyst (LPA) Anna Chan who met with the Office Administrator Binbin Zhao. Upon arrival, there were 14 preschool children and 2 staff.

A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

During a POC visit at the facility on 7/11/24, at around 12:00pm LPA observed Kody Nguyen ran to the office of the admin alone. Office Admin Zhao was in the office with LPA. Zhao walked the Kody back to the Casa classroom where the rest of the class is with Teacher Erica Rodriguez. Casa classroom is 2 doors down the hallway on the left side from the main entrance, and the admin office is inside the first door on the left, past the vacant classroom. Based on LPA’s observation, the doors of the classrooms (Casa and Toddlers classroom) were open.

Based on staff interview, Erica Rodriquez stated, the class finished lunch and almost time to transition to toddler room for nap. Teacher Erica stated when Kody heard the front door open, Kody ran out the classroom. Teacher Erica ran to the door of the classroom and tried to call Kody’s attention and also the attention of Ms Zhao and in the office and other Teacher Ms Kristina Robles in the toddler room. Teacher Erica stated she could not leave the class because there are no other teachers in the room. Then when Ms Zhao walked C1 back to the room, and Teacher Erica went back to the class and talked to Kody after. Ms Zhao notified the parents of Kody of what had happened during pick up.

Based on record review, child attendance data showed, C1 was picked up at 12:11pm on 7/11/24.

Exit interview conducted and report was reviewed with the Office Administrator Binbin Zhao. A notice of site visit was posted on the door and must remain posted for 30 days.
