LPA conducted a visit on 03/29/2024 and 04/10/2024. On 04/10/2024, LPA interviewed two (2) staff members. During the interview with staff, 2 out of 2 staff were able to reiterate infant safe sleep practices. 1 out of 2 stated the older infants who are able to crawl out of a crib sleep in the carpet areas on mats. During visit on 04/10/2024, LPA observed two (2) infants sleeping in the crib area and 6 (six) children sleeping on mats. The mats and cribs being used were observed to be following safe sleep practices. LPA also reviewed safe sleep logs that were fully completed by staff from 04/08/2024-04/12/2024.
LPA interviewed three (3) parents. Parents did not make any disclosures regarding the allegations of this complaint. LPA was not able to interview children due to age and language development.
Based on LPA’s observation, staff interviews and record review, the allegation staff did not follow safe sleep practices may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
Exit interview was conducted with Office Administrator Binbin “Vicky” Zhao. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
