25 | On 08/07/24, a case management inspection was conducted today by Licensing Program Analyst (LPA) Anna Chan and Licensing Program Manager (LPM) Martha Malane, who met with Teacher Damarys Chihuaque. Upon arrival there were 3 staff present and 8 infant children in care. The purpose of the inspection is to amend a report dated 07/24/2024 and to deliver discoveries from the 07/09/2024 inspection.
A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
LPA amended a version of report dated 07/24/2024.
In addition, on 07/09/2024, during record review, LPA discovered the previous facility director, Amynah Moloo is missing 8-hour Health Preventative with Lead certificate making Moloo unqualified to be a director.
In the areas evaluated, it was determined that the facility violated Health and Safety Code 1596.866(C)(i). See Type B deficiency cited on LIC809D
Exit interview was conducted with Teacher Damarys Chihuaque. The Notice of Site Visit was posted. Facility Representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
Appeal Rights and deficiency was explained. A copy of appeal rights (LIC 9058 1/16) was provided and their signatures on this form acknowledge receipt of these rights. The first level appeal is to Regional Manager, address is above on the report.
