Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of opening an infant/toddler and Toddler Option Program at this site. LPA met with the applicant/director, Leticia Noyes. LPA also met with acting directors, Stephanie Tjandra and Emily Cabuling who gave a tour of the entire facility to LPA. There are 6 classrooms have been designated for infants and toddlers and toddler option program. There is additional Piazza Room which is activity room with skylight high ceiling. There is an additional napping room or activities.

The applicant has requested to provide care and supervision for 24 infants and toddlers ages 3 to 18 months old in 3 rooms and 36 toddlers ages 18 to 36 months old in 3 rooms. Monday through Friday from 7:00 am to 6:00 pm.

LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements.

Parents sign in and out electronically at the entrance of the rooms. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities.

Isolation area is the Director’s office, located at the corner of the entrance of the building. There are additional playpen, and cots with beddings available for the sick child in the Director's office until the parents pick up the sick child/ren. There is staff bathroom in the lobby close to the director's office to be used for sick child/ren temporarily.

Safe sleep posters / SIDS

The following was observed in the Infant rooms and toddler option rooms
Age appropriate furniture and equipment’s
Toys, tactile equipment's
Rooms are well lighted with ventilation.
Infant rooms have napping area with cribs separated by Plexi glass or a separate room.
There is sufficient open area for the child to move
Changing table is placed inside one of the bathrooms in the classroom and at an arm's length of the sink, LPA discussed the positioning of the changing table for visual observation of children. The diaper changing table has padded surface no less than one inch tick with washable vinyl. It has raised sides on four sides at least 3 inches high. Storage cubbies are arranged to store personal belongings
There are 8 cribs in the napping area with mattress with tight fitting sheets in infant rooms 1 and 2, and cots are available for older children in the other 4 rooms. One crib shall be used by one infant at a time only. Flooring is wood and carpeting.
All Infant items will be provided by the facility. Formula for infants and lunch for older infants are provided by parent. Facility provides morning and afternoon snacks.
Refrigerator is available in infant rooms to store the milk and other items.
LPA reminded the director that all the food shall be labeled with the child's name and date.

LPA measured the assigned Rooms for infants and toddlers and toddler option children.
Room 1 is for infants ages 3 months to 12 months measures 508.61 square feet.
Room # 1 is equipped with a napping room measured 199.21 square feet which is not calculated in the activity are. There are 2 diaper changing tables in this room that are built in with a sink. There are 2 sinks in this room. Also there are 3 private sections nursery areas for parents to us

Room 2 is for infants ages 3 months to 12 months measures 397.76 square feet. Room # 2 is equipped with napping area separated by Plexie glass measures 109.2 which was not calculated in the activity space for this room. There are 2 toilets and 2 sinks in the restroom in this room. There is a diaper changing table built in with a sink in this room. There are 2 sinks inside the classroom.

Room 3 is for infants ages 12 months to 18 months measures 491.31 square feet. There are 2 toilets an two sinks in the restroom in this room. There is a changing table with no sink built in and is next to a sink within an arm's reach. There are 2 sinks inside this room.

Room 4 is for Toddlers from Toddler Option Program ages 18 month to 36 months old measures 464. 39 square feet which accommodates maximum 13 children. There is one toilet and one sink in the restroom in this room with additional potty chair. There is a diaper changing table in the restroom with no sink built in. The diaper changing table is close to a sink within the arm's reach.

Room 5 is for Toddlers from Toddler Option program ages 18 months to 36 months old measures 505.72 square feet which accommodates 14 toddlers. There is one toilet and one sink in the restroom in this room with additional potty chair. There is a diaper changing table in the restroom with no sink built in. The diaper changing table is close to a sink within the arm's reach.

Room 6 is for Toddlers from Toddler Option Programs ages 18 months to 36 months old measures 970.7 which accommodates 28 toddlers. There are 2 toilets and 2 sinks in the restroom in this room.
Large Room ( might be used as additional nap room or any other activity) 554.75 square feet.

Skylight high ceiling room for activity area measures 1624.41 square feet.
LPA discussed the positioning of the diaper changing tables for the staffs' visual observation of children while changing diapers. The diaper changing tables have padded surface no less than one inch tick with washable vinyl. They all have raised sides on four sides at least 3 inches high. The positions of diaper changing tables were changed for better supervision facing the rooms during inspection in 2 rooms. The other changing tables were built in to the ground. The applicant and acting directors stated they have always 3 teachers in a room. Staff are trained to call additional staff during changing diaper for better supervision. It was reminded never left a child unattended. The process of diaper changing was discussed and a hand out was posted by each diaper changing table.

Total indoor activity space is 5517.65 square feet divided by 35 square feet which will accommodate 157.65 (158 children).

Total sinks: 20 X 15= 300 children
Total toilets: 8 X 15 = 120 children
Total of Potty chair = 10 children

Outdoor activity space: 1820 square feet divided by 75 = 24.27 enough for 24 children) .
Playground is fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, outdoor activity toys, the area is covered with turf and soft cushioning. There is no high play structure on the outdoor activity space. There is one toilet and one sink on the outdoor space with a diaper changing table inside it. There is drinking fountain and water disperser for the outdoor area. The facility will send a waiver request to share the playground with Toddler Option children on different scheduling. The facility will also send a request for the requesting capacity of 60 children by using the additional skylight high ceiling area as outdoor space.
The facility has requested for 24 infants in rooms 1, 2, and 3 and 36 toddlers from Toddler Option Program in rooms 4, 5, and 6. There are additional Piazza Room (Skylight high ceiling activity area) and large room for additional nap room or other activities. The facility has enough indoor space to accommodate the requested capacity of 60 children.

LPA discussed the Safe sleep practice with the director, Leticia Noyes.

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold.

Fire clearance received from Orange County Fire Authority approving the requested capacity of 24 infants in rooms 1, 2, and 3 and 36 toddlers in rooms 4, 5, and 6. The director was reminded the maximum group of children in the toddler option program is 12 with 2 staff. If this number exceeds another group shall be created separate from the other group.
The director, Leticia Noyes is current on the required Immunization/ Pediatric CPR/ First training and has provided the completion certificate for the Preventive Health training (Nutrition and Lead component) and Mandated Child abuse reporting training.

Following items were discussed at the time of Exit interview.
Criminal Record Statement : The Director, Leticia Noyes was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Safe Sleep LPA discussed the safe sleep regulations with director, Leticia Noyes and discussed the Child Care Licensing Safe Sleep webpage at
https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the director, Leticia Noyes of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

Child Care Centers : LPA reviewed the LIC 311A, Records to Be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted. LPA discussed the checking on infants every 15 minutes during nap time and provided a sample of every 15 minutes check log. LPA also provided the LIC 9227 the infant sleeping plan form to the center representatives.

(2) Waiver request to accommodate 60 children for indoor capacity using additional Piazza Room (Skylight high ceiling room ).
