LPA met with the Director of Early and Expanded Education Dr. George Lopez and Applicant/Site Supervisor, Sylvia Ramirez Cuesta of Placentia - Yorba Linda Unified School District who gave LPA a tour of the facility. There are 3 classrooms( 502, 503, and 504) have been designated for preschool children ages 2.6 to 5 years old. The preschool program is located on the ground of Placentia-Yorba Linda Unified School District, Bryant Ranch Elementary School. The staff are employees of Placentia-Yorba Linda Unified School District. The applicant has requested to provide care and supervision for 71 preschool children ages 2.6 to 5 years old in rooms 502, 503, and 504 Monday through Friday from 7:00 am to 6:00 pm. The facility runs a full day Title 22 program. Parents sign in and out electronically at the entrance of the office. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities. Isolation area is the Preschool office in room 501 next to the children's classrooms. A staff person will take the child to the office and stays with the sick child until the parent picks up the sick child/ren. There is staff bathroom close to the office to be used for sick child/ren temporarily. LPA discussed the posting requirements including, but limited to, the following: · Facility License in public area (101160) · Emergency Disaster Plan (LIC 610) Continued on page 2

The following was observed in 3 classrooms:
Age appropriate furniture and equipment’s
Rooms are well lighted with ventilation.
There is sufficient open area for the child to move.
Storage cubbies are arranged to store personal belongings
Flooring is tile and will be covered with rugs.
Morning and afternoon snacks are provided by the facility and lunch will be provided by parents.
Water is provided in the children's water containers with their names on them. The facility refills the water for children if needed.
Children nap on cots. Beddings are provided by parents and are sent home to be washed by parents on a weekly basis.
LPA measured the assigned rooms for preschool:
Room # 502 is: 858 square feet
Room # 503 is: 858 square feet
Room # 504 is: 858 square feet
Total measurement of all 3 classrooms are 2574 square feet divided by 35 square feet = 73.54
(74 children).
Total sinks: 7 X 15= 105 children
Total toilets: 8 X 15 = 120 children

There are 4 toilets, and 2 sinks in the girls' bathroom and there are 4 toilets and 2 sinks in the boys' bathroom. There is one sink in each classrooms.
The facility representative was reminded that children shall be escorted to and from the restrooms and be under visual supervision at all times.
The restrooms belong to preschool and are not shared with any other programs at this site.

Outdoor activity space is the outdoor space in front of the classrooms for preschool children and will not be shared with any other programs at this site.
The measurement of the outdoor space is 9240 square feet divided by 75 square feet = 125.50

Outdoor space for preschool children is partially enclosed. The facility is working on appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, and outdoor activity toys such as bikes, sand tables and outdoor toys. There is no climbing structure on the outdoor space area. Shade is provided by trees and building structure.
Fire clearance received from Orange County Fire Authority approving the requested capacity of 71 preschool children in rooms 502, 503, and 504.

The Site Supervisor, Sylvia Ramirez Cuesta is current on the required Immunization, and Mandated Reporter Training. Her EMSA approved Pediatric CPR and Pediatric First aid training certificate expires 1/2024. The site supervisor has EMSA Preventative Health Training Certificate with lead poisoning and nutrition training.
Following items were discussed at the time of Exit interview.
LPA reviewed the LIC 311A, Records to Be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted.

The facility representative, Sylvia Ramirez Cuesta was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing Continued on page 4

clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on today’s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for 71 preschool children ages 2.6 to 5 years old in rooms 502, 503, and 504 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required. Exit interview conducted and report was reviewed with the applicant, Sylvia Ramirez Cuesta.
1- Final pictures of all the 3 classrooms after final set up showing the tall furniture secured.
2- Final pictures of the boys' and girls' restrooms after final remodeling.
3- Final pictures of the appropriate fencing to close off the outdoor space for preschool children.
4- Final picture of the outdoor space after being cleaned up and set up with outdoor toys.
