1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Page 1 of 4 Licensing Program Analysts (LPA) Mahnaz (Nancy) Malek, Mila Quinto, and Patricia Rivas conducted an unannounced on site pre-licensing inspection at this childcare center for the purpose of change of ownership from license number 304371028. LPAs met with the director, Lura Nixon who gave LPAs a tour of the entire facility. LPAs observed 9 infants with 4 staff and 32 toddlers with 6 staff in the classrooms and on the playground. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The applicant has requested to provide care and supervision to 12 infants ages 3 to 24 months old, and 36 toddlers ages 18 to 36 months old from Toddler Option Component in 3 rooms (Toddler Room 1, 2, and 3), Monday to Friday from 7:00 am to 6:00 pm. Parents sign in and out manually in front of each classroom. Isolation area is the director's office in the lobby area with an additional cot and bedding. The staff bathroom will be used if needed for the sick child until child is picked up. The following was observed in the classrooms:
Age appropriate furniture and equipment’s. Toys, tactile equipment's. Rooms are well lighted with ventilation.
There is sufficient open area for the child to move. Storage cubbies are arranged to store personal belongings. Flooring is wood. LPA discussed the positioning of the changing table for visual observation of children while using it and being within the arm's reach of a sink. The diaper changing table has padded surface no less than one inch tick with washable vinyl. It has raised sides on four sides at least 3 inches high.
Morning and afternoon snacks are provided by the facility and lunches are provided by the parents.
Water is provided in the children's water containers with their names on them. The facility refills the water for children if needed.
Children sleep on cribs and cots. Beddings are provided by the facility and are washed on a daily basis and as needed.
LPAs measured the assigned rooms for infants and toddler option:
Infant Room is: 424.91 square feet which is enough for 12 infants. (There is a napping room with 162.69 square feet which was not calculated in the activity area).
Toddler Option Room # 1 is: 461.75 square feet which is enough for 13 children
Toddler Option Room # 2 is: 453.91 square feet which is enough for 13 children
Toddler Option Room # 3 is: 482.46 square feet which is enough for 14 children
Total indoor space measurements for infant room and 3 toddler classrooms are: 4291.67 square feet divided by 35 square feet = 57.22 (57 children).
The facility has enough indoor space to cover the requested capacity of 12 infants and 36 toddlers,

Toddler option classrooms have their own restrooms.
Total measurement for the outdoor space:
Infant Playground is: 1237.62 square feet divided by 75 square feet =16 children
Toddler Option Playground is: 3054.51 divided by 75 square feet = 40 children
Outdoor space for both infants and toddler option children are fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, and outdoor activity toys. The area is covered with adequate of cushioning. There is a play structure on the outdoor activity space for toddlers with slide and swings for infants There are adequate cushioning (turf) around and under the falling zones. There is an exit gate leading to parking lot which is locked by a latch installed on the top of the gate. Shade is provided by several awnings and building structure.
Fire clearance received from Orange County Fire Authority approving the requested capacity of 12 infants and 36 toddlers.
The director, Lura Nixon is current on the required Immunization/Mandated Child abuse reporting training. Her EMSA Pediatric CPR/First training certificate expires 6/30/2024. She also has EMSA approved Preventive Health and Safety training with lead poisoning and nutrition training.
Following items were discussed at the time of Exit interview.
The facility representative, Lura Nixon was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with director, Lura Nixon and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the director, Lura Nixon of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LIC 9227 and infant sleep log for every 15 minutes were discussed.
LPAs reviewed the LIC 311A, Records to Be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted.
Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
To receive important license related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication. The facility representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Based on today’s indoor and outdoor measurements, the sinks and toilets availability, the request of the facility for 12 infants in infant room, and 36 toddlers in rooms Toddler Option 1, 2, and 3 will be final reviewed and discussed with management in the Regional Office and after receiving the following corrections. The facility will be notified if any additional information is required. Exit interview conducted and report was reviewed with the director, Lura Nixon.
