25 | Licensing Program Analysts (LPAs) Giselle Lucero and Kathy Trinh conducted an unannounced case management inspection, in response to a self-reported Unusual Incident Report (UIR) submitted to the Licensing Office on March 7, 2025. Upon arrival, LPA met with Interim Director Alicia Barajas.
At the time of the inspection, LPAs observed 3 infants in care with 1 staff.
A review of staff criminal record clearances conducted on this date confirmed that all facility staff and individuals requiring caregiver background checks have obtained the necessary criminal record and child abuse index clearances or exemptions.
The UIR, dated March 7, 2025, indicated that on Friday, March 5, 2025, Parent #1 (P1) contacted the facility to report that Child #1 (C1) had been observed with bruising on both calves and the lower abdominal area.
As part of the investigation, LPA interviewed four staff members and P1, obtained relevant medical documentation, and reviewed photographs provided.
During staff interviews, it was disclosed that C1 attended the facility for only one day. Staff reported that no bruising was observed on C1 during their attendance and C1 did not exhibit signs of discomfort.
Based on the information obtained through interviews and documentation review, LPA determined that there is insufficient evidence to support that any violations of Title 22 regulations pertaining to personal rights occurred in relation to the reported incident. Therefore, no deficiencies were cited during today's inspection.
An exit interview was conducted with Interim Director Alicia Barajas. A Notice of Site Visit was provided and posted at the facility.
