13 | Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced complaint visit to initiate an investigation into the above allegation. LPA was greeted and granted entry into the facility and explained the reason for the visit. LPA spoke with Administrator Maria Jasmine Avila by telephone.
During the course of the investigation, LPA interviewed Administrator and witness as well as reviewed and obtained pertinent documentation such as liability insurance. Regarding the allegation that facility does not have liability insurance, the investigation revealed the following: Facility has a current liability insurance policy effective 04/01/2024-04/01/2025. LPA confirmed with Insurance company that facility has current insurance and there has been no lapse in coverage. Based on interviews conducted and record review, the allegation is deemed unfounded, meaning the allegation was false, could not have happened and/or is without a reasonable basis. Exit interview conducted and a copy of this report was provided to facility representative. |
