13 | At 2:00pm on 2/18/2026, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Angie Thrasher to deliver findings to a complaint investigation into the above allegation. Upon arrivall, LPA observed a census of 12 infants supervised by 4 staff.
Throughout the course of the investigation, LPA conducted observation, interview, and record review relevant to the allegation that staff were not effectively preventing a child from scratching other children. In interviews, staff members stated their awareness of a child who scratched another child on multiple occassions. Staff interviews also provided corroborating accounts of a set strategy implemented to prevent the behavior, such as separating the two children into separate groups, providing a behavior coach to observe the child who scratched every day for 2-3 weeks, and introducing a supervision strategy of heightened shadowing. Record review showed this strategy to be documented. LPA observed sufficent supervision when visiting the facilities on 12/11/2025 and 2/18/2026. Based on available information, the allegation is found to be UNSUBSTANTIATED, meaning that although the allegation may have occurred or is valid, there is not a preponderance of evidence to provie it.
