Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Ali Ebert for an unannounced Case Management Inspection regarding an Unusual Incident Report.

Upon arrival, LPA observed 80 children, supervised by 10 staff members.

It was reported that on 04/02/2024, two children collided while on the playground, resulting in a split forehead to both children. Staff saw the collision happen and immediately attended to the children. LPA interviewed staff involved and reviewed footage of the incident. LPA determined there were no Title 22 deficiencies as a result of the incident.

An exit interview was conducted and this report reviewed with facility representative Ali Ebert. A Notice of Site Visit was provided and should remained posted for 30 days.
