32 | During record review, LPA observed staff to have the documents that were alleged to be missing. LPA did not observe record for a former staff 1, however there was no documentation of that staff not having the record when the previous annual was conducted on 10/23/2025.
LPA did not observe children left unsupervised during inspections, and staff interviews did not corroborate the allegation.
Because of this, the allegations are considered UNSUBSTANTIATED. These allegations may have happened or may be valid, but there is not a preponderance of evidence to prove that the violations occurred.
An exit interview was conducted, and this report was reviewed with licensee Barbara Warner. Appeal rights were provided. A Notice of Site Visit was provided and shall remain posted for 30 days. |
