On 3/18/21 at approximately 1:45pm, due to the COVID-19 pandemic, Licensing Program Analyst (LPA), Amanda Blesi, conducted a tele-inspection via Facetime and met with Director, Mallory Shepard, to deliver findings and conclude the complaint investigation of the above allegation. Census was 11 infants supervised by 3 staff. It was alleged that the infant classroom is operating out of ratio, particularly in the morning from 7am – 8am. Based on information obtained during interviews, review of parent sign in sheets, and review of staff clock in times, the infant room was out of ratio on 12/15/2020 from 7:35am to 7:45am. In addition, staff acknowledged there were a few days when parents would drop off their children earlier than anticipated causing the infant room to be out of ratio for short periods of time until another staff could be obtained. Director states this occurred a few times, and since then, she has adjusted staff schedules so there is adequate coverage. Based on the information obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. On 3/18/21 at am due to the COVID-19 pandemic, Licensing Program Analyst (LPA), Amanda Blesi, conducted a tele-inspection via Facetime and met with Director, Mallory Shepard, to deliver findings and conclude the complaint investigation of the above allegation. Census was 11 infants supervised by 3 staff.

The complainant alleged that a staff member handled day care children in a rough manner. During the investigation, LPA conducted interviews with both staff and parents. Based on the interviews, the preponderance of evidence standard was not met; therefore, the allegation is UNSUBSTANTIATED.

An exit interview was conducted and Notice of Site Visit was provided to be posted for 30 days. Facility evaluation report was emailed to Director and an email verification of receipt of report will be used in lieu of a signature on this report.

Due to the issuance of a Type A Citation during today's tele- inspection, a copy of this Licensing Report must be given to each existing parent by the end of today or next day child is in care, and to the parent of children enrolled over the next 12 months. In addition, a copy of the LIC 9224 Acknowledgement of Receipt of Licensing Reports must be signed by each parent and kept in each child's file.

Infant ratio: There shall be a ratio of one teacher for every four infants in attendance. This requirement was not met when on 12/15/2020, there was one teacher to 5 infants from 7:35am - 7:43am then two more infants arrived at 7:43am and 7:45am. This is an immediate health and safety risk to children in care.
