On July 23, 2025, Licensing Program Analyst (LPA) Lea Habtom met with director, Amber Williams, open and close a complaint. Upon arrival, LPA observed 10 children being supervised by 3 staff in the rockstars classroom. All staff present during today’s inspection have fingerprint clearances and associations.

The facility is commingling infants and preschool children

During the investigation, LPA Lea Habtom toured the facility, conducted observation and interviewed those pertinent to the investigation. It was alleged that the facility staff commingle infants and preschool children. Upon LPA arrival, there were 4 infants in the rockstar star classroom eating lunch with preschool children. There was also an incident that occured on June 23, 2025, where the child in question, was with a group of preschool children but was not 2 at the time of the incident. Interviews also revealed that infants, between the ages of 18 months and approaching 24 months, are combined with preschool children on a few occassions. Based on LPA observation and interviews the department has found that the allegation the facility is commingling infants and preschool children to be SUBSTANTIATED: meaning that the allegation is valid because the preponderance of the evidence standard has been met.

LPA Lea Habtom informed licensee Amber Williams that this report dated July 23, 2025, documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Lea Habtom informed the licensee Amber Williams to provide a copy of this licensing report dated July 23, 2025, that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

This report was reviewed with director, Amber Williams. A notice of site visit was provided to be posted for 30 days. Appeal rights provided.
