Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Courtney Williams for an unannounced Case Management Inspection regarding Unusual Incident Reports reported to LPA via email on 02/22/2024 and 02/27/2024.

LPA observed care and supervision of 90 children by 10 staff.

It was reported that on 02/07/2024, a parent that was viewing live footage of the facility alleged that she saw staff pin their child to the lunch table while they were sitting. Director spoke with the parent and the staff regarding the situation. Program Director was asked to review the footage of the incident and confirmed that the staff did not do anything wrong. LPA conducted interviews with both staff who were in the classroom at the time. LPA determined that the evidence provided does not indicate that staff did not honor the personal rights of the child in question.

It was reported that on 02/22/2024, a child briefly fainted while in care and hit their forehead. Staff immediately gave the child attention and contacted their parents. The child returned to consciousness during this time. The child was seen later at the doctor for a diagnosis. LPA determined that there was not a lack of supervision nor neglect.

LPA reminded director of reporting requirements outlined 101212(d).

An exit interview was conducted with facility representative Courtney Williams. A notice of site visit was provided and shall remain posted for 30 days.
