25 | At 9:00am on 2/12/2025, Licensing Program Analyst (LPA) Matthew Gallo arrived at the facility in relation to a separate complaint investigation for the Sterling Montessori facility at 821 Sterling Parkway. Upon arrival, LPA observed a census of 2 toddlers supervised by 2 staff. An infant arrived at 9:30am, to bring the total census to 3 children, consisting of 1 infant and 2 toddlers, supervised by 2 staff.
Upon the infant's arrival at 9:30am, the child was placed with the two toddlers in the toddler classroom. LPA confirmed through record review that the child was 15 months old. LPA left the 801 Sterling Parkway to return to 821 Sterling Parkway at approximately 10:00am. At 10:45am, LPA observed that the infant was playing with the toddlers on the infant center playground. At 12:30pm, when LPA delivered this report, the child was still co-mingling with the toddlers.
A Title 22 Deficiency is cited on the accompanying 809-D
Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided.
Exit interview conducted and report was reviewed with the licensee, Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days.
