13 | At 9:20am on 3/11/2026, Licensing Program Analyst (LPA) Matthew Gallo met with facility representative Paolo Sarmiento to deliver findings to a complaint investigation into the above allegation. During today's visit, LPA observed a census of 5 school age children supervised by 1 staff.
Throughout the course of the investigation, LPA conducted record review and interviews relevant to the allegation that staff did not provide parents with a licensing report documenting a Type A deficiency, as is required by regulation. On 3/5/2026, LPA reviewed 8 children's files and observed that 4 of the files did not include LIC9224 parent acknowledgements of receipt for the licensing reports of 2/13/2026 and 2/26/2026, both of which documented Type A deficiencies. The files that did feature the acknowledgement of receipt showed that the the reports were provided on 3/3/2026 or 3/4/2026. Through staff interview, LPA determined that the reports that were provided did not include all pages of the licensing report, but only the deficiency page. The preponderance of evidence standard has been met; therefore the allegation is SUBSTANTIATED.
