Licensing Program Analyst (LPA) Jeremey McClain met with facility representative Chelease Gardner to conduct an initial investigation for a complaint. LPA observed care and supervision of 10 preschool children by two staff and 5 infants by two staff.

It was alleged that the facility staff left a child unattended on the playground. LPA McClain addressed this allegation via Case Management inspection on 04/29/2025. Prior to that inspection, facility self-reported that on April 21, 2025, a child (C1) was outside on the fenced play yard without supervision from a staff. C1 was observed by a parent arriving to pick up their child, who then alerted staff. On 04/29/2025, facility was cited a Type A citation for Lack of Supervision. Based on prior investigation, the allegation is substantiated. No futher action will be taken regarding the allegation, as it was previously addressed.

An exit interview was conduected with facility represnetative Chelease Gardner. A Notice of Site Visit was provided and shall be posted for 30 days.
