25 | On August 26, 2025, Licensing Program Analyst (LPA) Stephanie Piring met with Licensee, Yvette Gonzales, for the purpose of an announced pre-licensing change of location inspection from facility 313625928. All individuals subject to criminal background review have obtained a criminal record clearance. Hours of operation will be: Monday through Thursday 07:30 AM to 05:30 PM.
The home is a two story single family home, which includes 4 bedroom, 3 bathrooms, kitchen, a living room, dining room, laundry room, garage, and backyard. Off-limits areas include: the entire upstairs, and garage. Licensee understands children may never enter off-limit areas. Fire Clearance was granted on 8/12/2025.
A health and safety inspection was conducted on the interior and exterior of the home. Toxic and hazardous items, sharp knives, and medications are inaccessible to children. LPA observed a functioning smoke detector, carbon monoxide detector, and a fully charged 2A10BC fire extinguisher within the home. LPA observed fireplace and stairs are properly barricaded to meet regulation. Licensee stated that there are no poisons in the home. LPA advised the licensee that if there are ever any poisons at the home, all poisons must be locked with a key lock or combination lock. Licenses stated there are no weapons in the home. There is an in-ground pool on the property which is surrounded by a see-through mesh fence that is at least 5 feet high. LPA observed the gate to self-close and self-latch. There are no windows or doors that provide direct access into the pool area. LPA did not observe a life saving ring, or a pole with body hook that is non telescopic at at least 12 feet long. LPA discussed regulations with licensee. LPA did observe a pool alarm installed, however the licensee states she ordered batteries for delivery.
Licensee has completed the required Preventative Health and Safety course with the Lead Poisoning Prevention training. Current EMSA approved pediatric CPR/First Aid trainings was completed and expires July 2026. Mandated Reporter Training was completed and expires July 2027. Applicant understands both CPR and mandated reporter training's’ must be completed every two years.
LPA discussed capacity and supervision with Licensee. Licensee understands that if an unusual incident occurs, licensing is to be notified via phone call, e-mail or fax within 24 hours and the Unusual Incident Report LIC 624 shall be submitted within 7 days to remain in compliance. LPA discussed Type A/B citations, open door policy, fire drills, children’s personal rights with the applicant. Licensee understands a current roster of children enrolled must be available and maintained for a period of three years, even after children are no longer attending.
