25 | On the date and time listed, Licensing Program Analyst (LPA) Nasha made an unannounced Plan of Correction (POC) visit this date to ensure that the deficiencies cited during an Annual Inspection conducted on 10/11/2022 has been corrected. LPA toured the facility, took census and met with the Center Director, Reichel Ainsworth, who was informed of the reason for the visit.
The facility was cited for deficiencies on 10/11/2022 during an annual inspection, and the corrections were not submitted by the Plan of Correction (POC) due date of 10/25/2022. The deficiencies that are still pending are as follows:
- CCR 101174(d) - Disaster drills shall be conducted at least every six months. During this visit, the Director submitted proof that a fire drill was conducted on 10/13/2022. LPA obtained a copy of the earthquake/fire drill log for record purposes.
- HSC 1596.8662(b)(1) – (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. (S2, S5, and S6 did not have copies of their Mandated Reporter Training Certificates). Proof of completion was submitted for S6 during today’s visit.
LPA is granting a one-week extension in order to allow more time for the remaining staff to complete their Mandated Reporter Training. The new POC due date is 11/10/2022. LPA advised the Director that if the remaining corrections are not received on or before 11/10/2022, an additional visit will be rendered with a possible citation being issued along with a civil penalty. |
