Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced complaint visit to the facility. LPA Malcore met with Administrator, Caroline Armstrong, and discussed the purpose of the visit.

Regarding the allegation, facility kitchen is not kept clean, LPA observed the kitchen clean and odor free.
Regarding the allegation, staff has inadequate record keeping for the residents, resident records were complete and available for LPA review.

Based on record review, the allegation above is Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
An exit interview was conducted with Administrator Armstrong and a copy of this report was provided to the Administrator at the conclusion of the visit.

Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced complaint visit to the facility. LPA Malcore met with Administrator, Caroline Armstrong, and discussed the purpose of the visit.
Regarding the allegation, staff had an inappropriate sleeping arrangement in the dining room, LPA conducted a tour of the facility. LPA observed a bed equipped with a mattress and bed linen next to the dining table. The administrator stated that she plans on having a partitian installed to have that area designated as a "staff room." LPA asked if anyone currently sleeps in the bed. The Administrator stated that staff #1 does sleep in the bed when the spare bedroom is occupied.
Based on LPA interviews and observations, the allegation above is Substantiated. A substantiated finding means that the preponderance of evidence standard has been met. Deficiencies are being cited per Title 22, Division 6 of The California Code of Regulations. An exit interview was conducted where reports LIC9099/LIC9099-D were discussed and copies with Appeal Rights were provided to the Administrator at the conclusion of the visit.
