On January 24 at 09:05 AM Licensing Program Analyst’s (LPAs), Courtnee Peebles and William Chancellor conducted an annual inspection as part of a compliance review. This is a combination childcare center and the other licensed programs are: Infant and School Age programs which was inspected on this date. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
· The facility is operating within the limits as stated on the license.
· Ratios are being met during this inspection with a total of four classrooms, 102 students and twelve staff
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards
· There are no weapons present at the facility as stated by Blanca Flores
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor activity space with public filtered water - and in the outdoor activity space by public filtered water
· Medications are stored where inaccessible to children, however During the facility tour and record review C1 had albuterol stored at the facility, however it was not in its original packaging and prescription. This poses a potential health and safety risk to children in care. A citation will be issued.

·Hazardous items are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous
· Poisons and toxins are locked and inaccessible to children
· All floors were observed to be clean and safe
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are enclosed by appropriate fences and free of hazards
· Outdoor activity areas are supplied with age and size appropriate equipment is not good condition. During the facility tour the playground had broken toys with visible sharp edges. Director Flores removed a broken bicycle and kitchen removing all hazards from the playground. This posed a potential health and safety risk to children in care. A citation will be issued. During the facility tour LPAs observed two empty rectangular planters outside that had sitting water accessible to children in care. This poses a potential health and safety risk to children in care. A citation will be issued.
· Food preparation area is not clean, free of litter and rubbish and free of rodents and other vermin. During the facility tour LPA observed a refrigerator with food and other items stuck to the bottom of the inside of the refrigerator. This poses a potential health and safety risk to children in care. A citation will be issued.
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed and meets regulation requirements
· Disaster drills are conducted at least every six months – last drill conducted on 12/12/2023

A review of staff and children's records were conducted as part of this evaluation.
Staffs records were found to be complete.
Children’s records were found to be complete during this inspection.
· Staff records review indicates that all staff present meet minimum qualifications for the position for which they were hired.
· A staff member is present with current Pediatric CPR/First Aid which expires on 08/15/2025
· Opening and closing staff member’s CPR/First Aid expires on 08/15/2025
· Director completed Health and Safety Training
· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. · Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request.


· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall
· The Licensee was informed of their reporting requirements and provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with assistant director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed assistant director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assistant director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

An exit interview was conducted, and this report was reviewed with the assistant director Blanca Flores. Appeal rights were discussed and provided during the exit interview.
