25 | Licensing Program Analysts (LPAs) Taadhimeka Zeigler and Justin Giese arrived at the facility on date and time listed. A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 06/17/2021. It indicates that on 06/14/2021, child #1 ran out of the classroom into the breezeway, but still behind the gate, unsupervised. Child #1 was intercepted by staff in the front office.
During today’s inspection, LPAs reviewed the files for staff #1 and child #1. LPAs also interviewed the Asst. Director, Daniela Nava. After the incident occurred, the facility placed staff #1 on administrative leave and conducted their own investigation resulting in the termination of staff #1.
Based on the information gathered, the following violation has been identified:

101229(a)(1) Responsibility for Providing Care and Supervision
The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to facility staff. |
