On May 24, 2023, at 9:20 AM, Licensing Program Analyst (LPA) Cindy Hamilton met with Childtime Children’s Center (CCC) Director Abbey Lewis to deliver the findings of the above stated allegation. On May 17, 2023, LPA Hamilton conducted a health and safety inspection of the facility and no immediate concerns were noted. During the investigation, LPA obtained copies of pertinent documents from children and staff records and conducted interviews with staff and parent.

On May 12, 2023, Community Care Licensing (CCL) received information stating staff dropped an infant in care. It was alleged that an infant was dropped twice and there was no disciplinary action for staff. Confidential interviews and records review confirmed the incident occurred on March 24, 2023. Confidential interviews resulted in conflicting information regarding the incident being provided to LPA. Confidential interviews disclosed that the child was being held at chest level when the child slipped or was dropped from the teacher’s arms resulting in child hitting head on the floor; whereas another confidential interview disclosed to LPA that the teacher was holding both of the child’s arms while the child was

standing, the teacher let one arm go and the child fell backward and hit child’s head on the floor. Confidential interview also disclosed that infant was being held by one arm as the staff attempted to assist another child with staff's other hand, resulting in the infant falling from staff's one-handed grasp to the floor. Confidential interviews also disclosed to LPA that the incident was an accident and no disciplinary action was taken. Confidential interviews revealed that the CCC's cameras were reviewed by Director and staff. LPA was unable to clarify how child fell from staff’s hold but confirmed that the child was dropped on at least one occasion.

Based on confidential interviews and records review, the preponderance of evidence has been met and the allegation that staff dropped an infant in care is substantiated. The facility is being cited for Title 22, Section 101223 (a)(2) Personal Rights which poses an immediate health, safety and/or personal rights risk to children in care.

An exit interview was conducted, a copy of this report and appeal rights were provided to Director. The Director was reminded that the Notice of Site visit must remain posted for 30 consecutive days.
