25 | On March 20, 2025, at 11:01AM, Licensing Program Analyst (LPA) William Chancellor and Licensing Program Managers (LPM’s) Pauline Beschorner and Carlos Martinez, conducted a Case Management visit, separate to an Informal Conference hosted in Riverside Child Care Office. LPA and LPM’s met with Licensee, Angel Anton and Director Candice Lawrence.
The purpose of the Case Management visit was to discuss recent Unusual Incident Reports (UIR’s). The first incident discussed, was an incident that occurred on February 5, 2025, involving child 1 (C1) being given the incorrect bottle that included medication. Interviews revealed that the initial error occurred with a staff member who is a parent that brought in a bottle with medication. Licensee and Director equally took accountability for there being other processes where errors occurred, that could have prevented the same bottle from being served to a different child. Licensee ensured all staff involved, have been talked to and appropriate disciplinary action has been taken.
The second UIR addressed, an incident that occurred in December 2024 and reported in February 2025. The incident involving a child choking on a grape and involving the Heimlich. During the visit, Licensee and Director disclosed that the child involved was taken to the doctor after school as precaution. Director assisted the parent in transporting the child to their pediatrician and forgot to report the incident to CCL.
The third incident, occurred on March 10, 2025, involving a child having an unknown allergic reaction. Parent of child 3 (C3) involved, disclosed once the child was home that evening, C3 was taken to the hospital after displaying allergic symptoms. Since the incident, the family has chosen to disenroll the child and Licensee confirmed that appropriate protocols are in place for children with allergies. All staff involved, ensured that C3 had no access to any snacks or foods that are an allergy.
There are no deficiencies being cited at this time.
An exit interview was conducted, a copy of this report, and appeal rights were reviewed with and provided to Director Candice Lawrence. A notice of site visit was also provided and must remain posted for 30 days.
