 On the above date and time, Licensing Program Analysts (LPA’s), Kelly Gerth and Kelli Waters, conducted an unannounced visit for a separate unrelated issue. During the visit, LPA’s were informed of an incident that occurred in December 2024. During the case management visit, LPA’s met with Director Candice Lawrence, toured the facility, took census, gathered documents, reviewed records and conducted staff interviews.

At this time, further information will be needed, upon completion of the review the outcome and/or recommendations will be provided.

An exit interview was conducted and a copy of this report and Notice of Site Visit was provided to Director Candice Lawrence and was reminded Notice of Site Visit must be posted for 30 days.
