Licensing Program Analyst (LPA) Tricia Danielson conducted this case management visit to address deficiencies discovered during the investigation of complaint control #10-CC-20240122121434.

Records reviewed during the investigation indicated the center did not submit Unusual Incident Reports (UIRs) to Community Care Licensing (CCL) for nine (9) incidents of physical aggression by Child #1(C1) during a period beginning June 4, 2019, to December 19, 2023. These incidents consisted of hitting, punching, and biting other students and one (1) attempt to hit a teacher. Additionally, two (2) of the nine (9) incidents consisted of C2 grabbing and/or touching the buttocks and/or genitals of other students. Furthermore, records reviewed failed to indicate the center addressed C1’s behavior which was a violation of the other children’s personal rights. Therefore, California Code of Regulations (Title 22), are being cited on the attached LIC 9099 D.

An exit interview was conducted, and a copy of this report was provided along with Appeal Rights, LIC9224- Acknowledgement of Receipt of Licensing Reports, and Notice of Site Visit.
