On April 07 2026, Licensing Program Analyst (LPA) Courtnee Peebles met with Discovery Isle (CCC) Director Melissa Almanza to deliver the findings regarding the allegation under investigation. As part of the investigation, LPA Peebles conducted confidential interviews and obtained documentation pertinent to the case.

Community Care Licensing (CCL) received an allegation stating that staff did not provide adequate supervision to day care children. Specifically, it was alleged that during a transition from outdoor to indoor activities, a child was left unattended outside while the remainder of the class went indoors.
To investigate the allegation, LPA conducted confidential interviews with staff. Information obtained revealed that during afternoon outdoor play, Classroom #3 was transitioning indoors with six (6) children and two staff members present. After the class settled indoors and diaper changes began, staff realized that Child 1 (C1) was not present. Staff immediately initiated a search, and C1 was located on the playground by a staff member from a neighboring classroom. Based on interviews, C1 was unsupervised

for approximately 2–3 minutes. Further information indicated that staff to child ratios and supervision are typically maintained, and classroom transitions are generally conducted with both assigned staff present. It was also reported that an additional staff member usually assists Classroom #3 during transitions due to the age group. However, on the day of the incident, staff did not complete a name to face count, which contributed to the lapse in supervision.

Staff consistently reported that required ratios and supervision procedures are maintained during transitions, aside from this isolated incident. While the information obtained confirms that C1 was briefly left unattended, there is insufficient evidence to support a pattern of inadequate supervision. Therefore, the allegation is deemed unsubstantiated.

An exit interview was conducted with Director Melissa Almanza. The report was reviewed, and a copy was provided. Appeal rights were discussed and issued at the time of the exit interview.

A Notice of Site Visit was also issued and must remain posted in a location visible to the public for 30 consecutive days.
