On 03/15/2023, Licensing Program Analyst (LPA) Lorena Valenzuela conducted a follow up visit in response to the receipt of an unusual incident report (UIR) from the facility. LPA met with Vickie Fluellen, Teacher. The UIR was received by the licensing agency on 01/31/2023.
LPA interviewed Staff #4. Further information will be needed. Upon completion of the review, the outcome and/or recommendations will be provided to the facility.

An exit interview was conducted and a copy of this report was provided to Vickie Fluellen, Teacher.
