25 | On the date and time listed, Licensing Program Analysts (LPAs) Perla Ordones and Samuel Lopez arrived at the facility for the purposes of conducting a Case Management inspection in response to the receipt of an Unusual Incident Report (UIR) that was submitted by the facility on 05/09/2024. The UIR was investigated by Community Care Licensing Investigation Branch Investigator Dennis Douglas who was assigned the case in May 2024. LPAs were granted entry into the home by Licensee Maria “Teresa” Navarro. LPAs explained the purpose of today's inspection, completed a facility tour, and obtained a census.
It was reported that on 05/08/2024, it was alleged that an inappropriate behavior occurred at the facility, involving a day-care child. Pertinent parties were interviewed by Investigator Douglas but none were able to corroborate the stated allegation. For these reasons, no violations have been found as of this time.
Based on information obtained during the investigation, it has been determined there was no violations of Title 22 Regulations pertaining to the reported incident at this time. No deficiency cited at this time.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
