During a previous visit and today’s visit, LPA observed appropriate ratios were maintained by staff. LPA observed staff stationed in different areas of the classroom ensuring adequate supervision. Interviews conducted disclosed staff also station in different areas of the outdoor play area to ensure adequate supervision. There were no disclosures made regarding children in care being left unattended. Information obtained indicated staff communicate any injuries and incidents involving children in care with parents and management. It was revealed parents are notified of any injuries their children sustained while in care via telephone, through an incident report, or at pick up. Interviews did not disclose consistent statements regarding documentation of other various incidents that could occur, such as suspected child abuse, a child wandering away from the facility, or a child left unattended. It is determined there was not sufficient information evident to support the above allegations.
Based on observations made at the facility and information obtained during interviews, it is determined that the allegations could not be substantiated or dismissed. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED.
An exit interview was conducted and report was reviewed with the site supervisor, Teresa Garcia. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
