On October 09, 2023, at 9:04 AM, Licensing Program Analyst (LPA) Anastasia Flores, met with Facility Director, Desiree Andrews for the purpose of delivering the finding of the above stated violations. On August 15, 2023, at 3:09 PM, LPA conducted a health and safety inspection, and no immediate concerns were observed. Copies of child roster, staff roster, facility schedule, and staff dress code were obtained. LPA conducted interviews with five out of five staff.

On August 10, 2023, this agency received allegations that day care children sustained injuries due to staff neglect, staff screams at day care children, and staff did not notify day care children’s parents of illness. Confidential interviews disclosed that children on more than one occasion, child #1 (C1) only name provided, were injured at the day care due to staff always on their phones and lack of supervision. Confidential interviews disclosed that C1received multiple injuries on one day at the day care and no specifics were given to parents at time of pick-up. Interview with facility director, denied allegation that children are injured due to lack of supervision, and staff are not allowed to be on their phones while at work.

It was reported that staff have been heard yelling at the children. Confidential interviews disclosed that S2 does talk in a loud tone, but has not been heard screaming at the children. Other confidential interviews disclosed that staff #2 would scream at the children resulting in children being afraid to return to S2’s classroom. Other confidential interviews revealed that S2 does talk in a loud tone but has not been heard screaming at children in care. Confidential interviews disclosed that the Director allowed a child, no specifics given, a child to stay at school with a fever resulting and staff getting sick a few days later. Interview with director denied allegation, stating the parents are called immediately to pick up their children when they are sick. Other confidential interviews revealed that when children are sick the parents are called to pick up their children.

Based on interviews and lack of corroborating evidence from the reporting party, although the allegations that day care children sustained injuries due to staff neglect, staff screams at day care children and staff did not notify day care children’s parents of illness may have happened or is valid, there is not a preponderance of evidence to prove the allegations did or did not occur, therefore the allegations are unsubstantiated.
An exit interview was conducted, appeal rights discussed and was handed to Facility Director, Desiree Andrews.

A Notice of Site Visit was provided and must remain posted for 30 days.
