On October 2, 2024, at 9:00 am, Licensing Program Analyst (LPA) Cindy Hamilton met with Stepping Stones Preschool Director Desiree Andrews to deliver the findings for the above allegation. On August 15, 2024 and September 18, 2024, LPA conducted health and safety inspections of the facility, and no immediate health and/or safety concerns were noted. During the investigation, LPA conducted interviews with two staff, one child, and another relevant party. LPA also obtained copies of pertinent documentation from facility and children’s files. The child identified as the victim and/or witness was not interviewed due to child did not wish to be interviewed.

On August 9, 2024, the Department received information that a child sustained unexplained injuries in care. It was alleged that a child received multiple injuries on 08/02, 08/05 and 08/06/2024 while in care and Director was unaware of the injury that occurred on 8/2/2024. Confidential interviews resulted in conflicting information being disclosed to LPA. Confidential interview disclosed that the child was injured right before being picked up on 08/02/2024 and parent declined having an incident report completed; whereas

another confidential interview disclosed that parent was verbally advised of the details regarding the injury on 8/2/2024 and no written documentation was mentioned or offered to the parent. In addition, a confidential interview disclosed the director was made aware of the incident and the injury on 8/2/24. Confidential interviews and observation revealed that CCC uses correct ratio regulated by Title 22 and provides proper supervision. In addition, all the incidents were witnessed by a teacher. Records review revealed that the child in question had incident reports for injuries obtained on 08/05/2024, 08/06/2024 and 08/13/2024 that provided explanations for the injuries received by the child. Confidential interviews revealed that the child in question is very active and climbs on the play equipment inside the classroom; whereas another interview disclosed that the child does not climb on objects at all. LPA was unable to interview the child involved and the teacher present on 8/2/2024 to corroborate the allegation.

Based on interviews, observation and records review, the allegation child sustained unexplained injuries in care, may have occurred, however is not supported or proven by evidence. Therefore, the allegation is unsubstantiated at this time.

A copy of this report, appeal rights and Notice of Site Visit were provided to director. The director was reminded that the Notice of Site Visit must be posted for 30 days and was posted prior to LPA leaving the CCC.
