25 | On the date and time listed, Licensing Program Analysts (LPAs) Perla Ordones and Taityana Benson conducted a case management visit in response to the receipt of an Unusual Incident Report (UIR) from the facility. The UIR was received by the licensing agency on 02/12/2024. The UIR documents an incident that involved a day-care child that received an injury to their left pinky finger due their hand getting caught in a door hinge at the facility.
Upon arrival, LPAs met with Director Monique Garcia and stated the purpose of the visit. Facility was toured, census was taken, records were reviewed, and interviews were conducted with pertinent parties. It was disclosed that the subject child had went behind a staff member who was receiving another child into the Mountain Classroom when they stuck their hand near the door hinge as the door was closing. Staff applied first aid immediately and contacted the subject child’s authorized representative promptly. LPAs learned that door hinge protectors were installed in all preschool classrooms to prevent further incidents and that new additions to protocols were implemented.
Based on the information gathered, there were no violations of Title 22 regulations identified at this time in regard to the UIR.
There were no deficiencies cited at this time.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
