25 | On the date and time listed, Licensing Program Analyst (LPA) Perla Ordones arrived at the facility for the purposes of conducting a case management inspection in response to the receipt of an Unusual Incident Report (UIR) that was submitted by the facility on 04/22/2024. LPA explained the purpose of today's inspection, completed a facility tour, and obtained a census.
LPA made observations, reviewed pertinent documentation, and conducted interviews with pertinent parties related to the self reported incident. It was reported that on 04/19/2024, a day-care child received an injury from an unknown source after placing their hand into a hole in the ground. Interviews revealed that first aid was immediately applied to the day-care child and authorized representatives were immediately informed. LPA was informed that grounds keepers are employed by the facility and check the play ground area every Friday before children arrive and as needed. The Director stated that both the grounds keeper and staff ensure that the holes are filled as soon as they appear. Interviews also revealed that staff remind the children to not put their hands in the ground as they can get injured.
Based on the information gathered, there were no violations of Title 22 regulations identified at this time in regard to the UIR. There were no deficiencies cited at this time.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
