32 | In regards to the allegation that staff did not prevent day care child(ren) from being harmed by another child, it was also reported that staff are not addressing a child's challenging behaviors. Further, it was reported that this resulted in another child being scratched in the face and hit multiple times.
LPAs conducted interviews with pertinent parties who disclosed that staff members have tried different methods to prevent scratching such as having the child wear mittens and request to have their nails trimmed. However, incidents where children were scratched by another day-care child still occurred. Furthermore, LPAs obtained documentation that showed over 20 facility incident reports regarding the child scratching others. LPAs noted that the documentation dates back to January of 2024.
Based on interviews of pertinent individuals that were conducted, and a review of additional pertinent information obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated.

LPAs Raymond Moorehead and Perla Ordones informed Assistant Director Jennifer McClintock that this report dated 05/07/2025 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of the children in care.
Also, LPAs Raymond Moorehead and Perla Ordones informed Assistant Director Jennifer McClintock to provide a copy of this licensing report dated 05/07/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgment of Receipt of Licensing Reports (LIC 9224), or other written statement, must be placed in the child’s file for verification.
A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a civil penalty of $100.00.
