Licensing Program Anaylysts (LPA's) Keely Messerschmidt and James Wilkerson arrived at this facility to investigate the above allegations. LPA's toured the facility and conducted a census. LPA's met with the acting Director Gabriela Padilla and discussed the allegations.

1. Staff left daycare child unattended: it was alleged that a child was left outside on the playground unattended for approximately 2 minutes. The Unusual Incident Report was received on March 14th stating that a child was left outside unattended and an interview with S1 indicated that child was indeed left outside. Based on the Unusual Incident Report S1 own admission this allegation is substantiated. See LIC 9099-D for defiencies cited. This is a second violation (10/15/22) for Title 22 Regulation 101229 (a) (1), this will result in a Civil Penalty for $250.00.

Acting Director Gabriela Padilla agrees to submit in writing a plan on how they will remain in compliance with Title 22 Regulations on Supervision. Ms. Padilla was reminded of the $100.00 Civil Penalty for every day the POC is late, per California Health & Safety Code Section 1548(b), 1568.0822(b), 1569.49(b), 1596.99(b) or 1597.58(b).

Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility for the next 12 months. A Civil Penalty has been assessed on this visit.

Payment is due when billed and the check(s) or money orders shall be made payable to the “California Department of Social Services”. YOU WILL RECEIVE AN INVOICE IN THE MAIL. DO NOT SEND MONEY UNTIL YOU RECEIVE YOUR INVOICE. DO NOT SEND CASH.

The Notice of Site Visit and Type A Deficiencies from today’s visit must be posted for 30 days. Failure to keep these posted for the entire 30 days will result in an immediate $100 civil penalty for each.


2. Unqualified staff: it was alleged that facility was operating with unqualified staff. The facilities preschool program operates with a toddler component. LPA's reviewed staff files and observed that an opening teacher did not have the adequate qualifications. It was disclosed that this staff member was left alone with children. See LIC 9099-D

An exit interview was conducted, Appeal Rights were discussed and provided along with a Notice of Site Visit, copy of LIC 9224 (AB 633) and a copy of this report was provided to this facility on this date.
