13 | On December 15, 2022 at 1:16 PM, Licensing Program Analyst (LPA) Anastasia Flore met with Site Director, Mary Velasquez to deliver the findings on the above allegations. On November 22, 2022 at 12:31 PM, LPA conducted a health and safety inspection of the facility and not immediate concerns were observed. Copies of facility staff roster was obtained, and records were reviewed. Interviews were conducted with three out of three staff.
On November 18, 2022 this agency received allegation that facility staff throw rubber scraps from the playground at the day care children. It was reported that staff #1, staff #2 (S1, S2) threw rubber scraps from the playground at the children stating, “I was trying to hit them on the head,” on at least one occasion. Interview with S1, S2 denied throwing rubber scraps at the children in care or saying anything demeaning to the children. Interview with staff #1 disclosed that there is one child that kicks the rubber scraps at the teachers in a playful manner and the staff kick it back towards the child playfully never hitting the child.
