On the above date and time Licensing Program Analysts (LPAs) Susan Brewer and Patricia Berry, arrived at the facility for the purpose of continuing a complaint investigation for the above allegation and to deliver findings. LPAs were greeted and granted entry by Director Karent Brazzill. The LPAs took a census of 17 children in care and supervised by 3 staff.

On 10/29/2024 the LPAs Susan Brewer and Patricia Berry met with a facility staff and Director Karen Brazzill to initiate the complaint and discuss the above allegation of Personal Rights, however the LPAs were unable to conduct the investigation and the licensee was informed that the investigation would resume at a later date. On today's date the LPAs continued the investigation. The LPAs obtained facility records, made observations and conducted interviews with pertinent parties regarding the allegation.

It was alleged that facility staff left a day care child soiled for an extended period: LPAs made observations, gathered facility records and conducted interviews with pertinent parties which revealed the following. Interviews conducted with pertinent parties described procedures for diaper changing and potty training of preschool children. LPAs conducted a review of facility records, which document dates and times for each diaper change performed by staff with status descriptions for subject children.

Although records indicate that subject children were reported to have accidents requiring a diaper change or a change of clothing consistent with the activities scheduled for preschool children, the LPAs observed inconsistencies in facility records. Facility records obtained and reviewed by the LPAs conflict in time lapses logged for diaper changes of subject preschool children, the activity schedule for when napping ends and when the changing of diapers begins for some subject children. Based on conflicting information gathered through facility records, interviews conducted with pertinent parties and observations made, the allegation is determined to be Unsubstantiated

Although the allegation that facility staff left a day care child soiled for an extended period may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.

No citations were issued.

No civil penalties were issued.

Exit interview conducted and report was reviewed an copy left with the Director Karen Brazzill.

A Notice of Site Visit was given and must remain posted for 30 days.
