25 | On March 27, 2024, at 9:45 AM, Licensing Program Analyst’s (LPAs), Anastasia Flores, Ana Noble, and Gabriela Hernandez, arrived for the purpose of change in capacity for school age program. LPAs toured the facility, during the tour of facility, LPAs observed an enclosed rod iron fencing outside the infant classroom. The enclosed area had soft infant items for climbing and crawling inside the enclosure. Licensee informed LPAs she had the enclosure made out of extra rod iron fencing from the fence that was taken down around the facility. LPAs informed Licensee, to inform our agency, prior to any kind of construction to the facility. LPAs informed licensee during inspection, the purpose of informing Community Care Licensing prior to any and all changes to the facility that may endanger the safety of the children in care and handed licensee a copy of Title 22 regulations for future records. During the inspection, LPAs toured the outdoor activity area for school age children. LPAs observed there to be wooden logs with splinters and left over plywood leaning against the fence, dry brush, and debris in and around the playground area, easily accessible to children in care.
The facility is being cited for Title 22, section: 101238.2(d)(2) Outdoor Activity Space. See 809D for deficiency. Licensee was informed if if there are any further changes completed in the facility, without notifying the department, licensee would be cited and an office conference will be held with our department.
A copy of this report, LIC809D, and appeal rights were discussed and handed to licensee, Gayle Clark. A notice of Site Visit was handed to licensee and must remain posted for 30 days. |
