25 | On 7/29/2024 at 11:30 AM, Licensing Program Analysts (LPA) Claudia Caywood arrived at the facility to conduct a Case Management Increase of Capacity. Upon arrival, LPA was met by Licensee, Malektaj Esteaneh. LPA stated to the licensee the purpose of the visit. During the facility check, LPA toured the facility (inside and outside), and took a census. There were three children present on this day. LPA obtained translation services by Sogand ID #3957318.
The licensee initiated a capacity increase with the department and fire clearance granted on 5/31/24. LPA conducted a follow up visit to clear outstanding POC’s from a previous capacity increase inspection. Licensee cleared all but one POC. Children’s documentation still needs to be obtained. Increase of capacity is still being evaluated.
During today’s inspection visit, LPA observed two children napping with pacifiers clipped to their t-shirts and another child napping with a bottle in hand. LPA asked translator to explain to licensee these are department violations and can turn in to deficiencies. LPA asked translator to explain to the licensee crib mattresses need fitted sheets when in use. LPA had translator explain to licensee the need for an office visit to which she agreed to.
An exit interview was conducted, and report was reviewed with the licensee, Malektaj Esteaneh. A Notice of Site Visit was issued and is to be posted in a prominent location at the facility for the next 30 days.
