25 | On 10/03/2023 at 9:00 AM, Licensing Program Analyst (LPA) Susan Brewer, arrived announced for the purpose of conducting a Pre-Licensing Inspection. The applicant Sarah Davis, was previously licensed and has relocated to a new facility. The LPA was greeted and granted entry to tour the facility, inside and out, records were reviewed and the following was observed and/or discussed: Present during the inspection, were the Licensee Applicant and two other adults. The applicant originally applied to be licensed with another adult resident and owner of the home Beth Levario. However, on today’s date, the co-applicant Beth Levario, requested to be removed from the LIC279 Application form, to opt out of becoming licensed at the facility. The LIC279 Application form was updated during the inspection.
Normal days and hours of operation are: Monday – Friday, 7:00 AM to 5:30 PM.
OFF-LIMIT AREAS INCLUDE: Garage, Laundry Room, Entire 2nd Floor and Backyard.
· Appropriate fire extinguisher, smoke detector and carbon monoxide detector are present and were tested by the applicant during this inspection on 10/03/2023.
· A fire clearance was approved for a Large capacity on 09/28/2023, by the Riverside City Fire Dept.
· All hazardous items located in the garage and laundry room, leading to the garage.
· Toxins are not yet observed to be locked in the garage or laundry room.
· Guns or weapons are present as of this 10/03/2023. APPLICANT SARAH DAVIS, UNDERSTANDS ALL GUNS, WEAPONS AND AMMUNITION MUST BE KEY-LOCKED SEPARATELY AND MADE INACCESSIBLE PER TITLE 22 REGULATIONS. Per applicant, were unable to provide the location and access for the LPA S.Brewer, in order to verify the compliance of firearm and storage. LPA did observe ammunition to be under combination lock. |
