La'Quanda Milton was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Before licensure, the following needs to be corrected/completed:
1. Pool fencing and door/gate needs to be modified to meet Title 22 requirements
2. Palm tree located in the back yard (east side), needs to be removed or made inaccessible
3. Parent Poster Board needs to be posted (LIC 610A, LIC 999A, PUB 394, Car Seat law (if applicable))
4. Applicant requires to complete the move in from her previous place of residence
Once all corrections have been made, with proof sent to licensing, the application for a Large Family Child Care Home will be submitted for approval with a maximum capacity of 12, or 14 with parent notification. As agreed, upon by the licensee, all corrections are due within 30 days. If not received within 30 days from the date of this report, the application may be withdrawn. |
