25 | Licensing Program Analyst (LPA) Claudia Caywood arrived at the facility to conduct a Case Management visit in response to the receipt of an Unusual Incident Report (UIR) from the facility dated
8/22/2025. It documents an incident involving a child who slipped and fell off a play structure resulting in a broken arm.
At the time of the case management inspection, LPA toured the facility, took census, and met with Jet’aime Abeyta, Site Lead. Interviews were conducted to obtain details regarding the reported incident. During the facility tour the area in which the incident occurred was observed.
Interviews revealed a child was playing on the outdoor play structure when they fell and hurt their arm. Upon inspection of the child’s arm by facility staff, it was observed the child’s arm had a raised bump. First aid was applied to the arm, authorized representatives immediately notified, and child was taken to receive medical care.
Based on the information gathered, the facility acted appropriately, and no violations have been identified. Facility staff applied first aid and immediately notified authorized representatives and reported the incident in a timely manner.
A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
