On date and time listed, Licensing Program Analyst (LPA) Perla Ordones arrived at the facility to conduct a pre-licensing inspection. Present during this inspection was: Applicant Yesenia Casillas. LPA toured the facility, inside and out and the following was observed and/or discussed:

Normal days and hours of operation are: Monday – Friday; 06:00AM – 05:00PM.

OFF-LIMIT AREAS INCLUDE: Bedroom 2, Master bedroom closet, Master bedroom bathroom, laundry room, backyard building, shed in backyard, backyard, carport, and front yard.

· Appropriate fire extinguisher, smoke detector and carbon monoxide detector are present and were tested by the applicant during this inspection. Fire extinguisher, smoke detector and carbon monoxide detector are in working order.
· All hazardous items are inaccessible.
· Storage of poisons/toxins are NOT inaccessible to children and locked. During facility tour, LPA observed that several poisons/toxins were not locked in a key locked area.
· Weapons are not stored according to Title 22. During facility tour, LPA observed that ammunition was not locked per Title 22 regulations. Applicant understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations.
· This is a single-story home.
· Fireplace is properly screened to prevent access by children.
· The applicant, or licensee provided proof of control of property.
· Facility Sketch and Emergency Disaster Plan are posted.
· Mandated Reporter Training completed on 12/14/2023.

· Pediatric CPR and First Aid Card completed on 12/17/2023.
· Health & Safety Certificate - completed on 01/17/2024.
· No bodies of water at this time. Applicant understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Clean, safe and age-appropriate toys.
· There are toxic plants observed at this time. During facility tour, LPA observed three poisons plants present in the dining room and kitchen area.

· The Applicant was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov

· Resident and/or staff records reviewed on 04/15/2024 indicate that all adults who require caregiver background checks have received all required clearances or exemptions.

· The Applicant can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov. Applicant was provided information regarding Guardian.

The following was discussed with the applicant(s):

- Pre-Licensing Visit Packet provided.
- Failure to meet the posting requirements shall result in an immediate $100 civil penalty.
- Documentation of fire & earthquake drills to be conducted every six months.
- Responsibilities of being a mandated reporter.
- Responsibility to know the regulations for anyone providing care.
- Inaccessibility of hazards must be constantly reassessed depending on the children in care.
- Current facility’s phone numbers must always be on file with the licensing office.
- Baby walkers, baby-bouncer, and walker-jumpers are prohibited (H&S 1500.86(a)(4)).

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The Duty Officer is available to answer questions Monday – Friday; 8:00am to 5:00pm at:
1-844-LET-US-NO (1-844-538-8766) and/or 951-782-4200

Before licensure, the following needs to be corrected/completed:
- Poisonous/toxic plants must be removed.
- Ammunition must be locked according to Title 22 regulations.
- Poisons/toxins must be stored in a key locked area.

Once all corrections have been verified, the application for a Small Family Child Care Home will be submitted for approval with a maximum capacity of 6, or 8 with parent notification. Applicant advised that all corrections are due within 30 days or the application may be withdrawn.

On this date, 04/15/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
