On date and time listed, Licensing Program Analyst (LPA) Perla Ordonez and Taityana Benson arrived at the facility to conduct an annual inspection as part of a compliance review. LPAs met with Licensee Xerian Williams who was present at the time. Please note that Licensee Tatiana Williams arrived at the facility at 11:14AM. LPAs toured the facility, inside and out, records were reviewed, and the following was observed and/or discussed:

Normal days and hours of operation are: Sunday – Saturday; 05:30AM – 10:30PM.

OFF-LIMIT AREAS INCLUDE: Entire second floor, bedroom 1, and garage.

The facility is operating within the licensed capacity and appropriate ratios.

· Appropriate supervision provided during this inspection.
· A working telephone is present and current number on file.
· Appropriate fire extinguisher, smoke detector and carbon monoxide detector present and were tested by the Licensee during this inspection.
· Fireplace is properly screened to prevent access by children.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

· All hazardous items are not stored inaccessible to children. During facility tour, LPAs observed several hazards in the on-limits areas including a broken screen door/window in the side yard as well as plastic bags, a sharp object, and cleaners in the on-limits kitchen area.
· During inspection, LPAs observed that a baby bouncer was present however LPAs observed that all day-care children present would not be able to utilize the bouncer due to their age. Licensees removed baby bouncer from premise during visit.
· Weapons are not present. Licensee understands all guns, weapons and ammunition must be key locked separately and made inaccessible per Title 22 Regulations.
· This is a two story home. Stairs were not barricaded. During inspection, LPAs observed that gate barricading stairs was left open multiple times during the inspection and day-care children under 5 years old were present.
· Verification of control of property on file.
· Facility Sketch, Emergency Disaster Plan & Notification of Parent’s Rights poster are posted.
· Mandated Reporter Child Care Providers Training completed on 01/21/2025 for Licensee Tatiana Williams and 02/29/2024 for Licensee Xerian Williams.
· EMSA Pediatric CPR and First Aid Card completed on 05/17/2025 for Licensee Tatiana Williams and 05/17/2025 for Licensee Xerian Williams.
· Health & Safety Certificate – completed on 06/02/2023 for Licensee Tatiana Williams and 04/11/2022 for Licensee Xerian Williams.
· No bodies of water at this time. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 Regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Clean, safe and age appropriate toys not present. During facility tour, LPAs observed two play structures with missing handlebars and seats, a basketball hoop in poor repair, and a play structure with a broken pole.
· LPAs advised Licensees on Assembly Bill (AB) 2084 and provided a copy of AB2084 to Licensees.
· Current roster not on file. During record review, LPAs observed that roster was not up to date.
· Documentation of fire and disaster drills on file – Last drill conducted on 02/21/2025. LPAs advised Licensees on the requirement to conduct fire/disaster drills ever six months. LPAs advised Licensees on Operation of a Family Child Care Home regulations.

· Children’s records are not complete. During record review, LPAs observed that C3, C4, C5, C6, and C7 were missing completed Identification And Emergency Information forms (LIC700s). Additionally, LPAs observed that C5 was missing proof of the Consent For Emergency Medical Treatment (LIC627), Family Child Care - Notification Of Parents' Rights (LIC995A), Affidavit Regarding Liability Insurance For Family Child Care Home (LIC282), Parent Notification Additional Children in Care (LIC9150), and immunizations. Furthermore, during record review, LPAs observed that records for children disenrolled April of 2025 were not kept on file.
· Employee’s records are not complete. During record review, LPAs observed that S1 did not have proof of the measles, pertussis, or influenza immunization.

· The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov

· Resident and/or staff records reviewed on 07/22/2025 indicate that all adults who require caregiver background checks have NOT received all required clearances or exemptions. Upon LPAs arrival, LPAs were informed that S1, who is not currently associated to the facility, had left the facility for a few hours with C1 while Licensees were present at the facility. According to information LPAs obtained, S1 left with C1 at approximately 10:30AM and returned with C1 at approximately 12:52PM.

· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations862@dss.ca.gov

LPAs discussed the safe sleep regulations with Licensees and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPAs also informed Licensees of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensees were reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Licensees were informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Also, LPA Perla Ordonez and Taityana Benson informed Licensee Xerian Williams and Licensee Tatiana Williams to provide a copy of this licensing report dated 07/22/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

During the exit interview, Licensee Xerian Williams and Licensee Tatiana Williams, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Exit interview conducted and report was reviewed with the Licensee Xerian Williams and Licensee Tatiana Williams.
