On 01/23/2025 at 11:25 am , Licensing Program Analyst (LPA's), Griselda Castellon and Diana Brasel conducted an announced Pre-Licensing inspection for a New Single License. Upon arrival, LPAs met with Azizi Gates owner/ administrator. Azizi Gates requested to be licensed for 36 preschool children ages 2-5 in the Orange Room, Green Room, and Blue Room, and 5 infants ages 6 weeks - 2 years in the in the purple room. During the visit the applicant advised LPA's that she would like to withdraw the infant requested component. LPA's obtained an updated LIC 200A and a written request to withdraw the infant component, the purple room will be off limits. The Hours of operation will be Monday through Friday 7:00 am to 6:00 pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPAs
informed Ms. Gates that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Ms. Gates stated that they will be in locked in the office cabinet. A fully equipped first aid kit is located in the office. The Carbon monoxide detector is located in the kitchen area. All required licensing documents were observed posted in the lobby. Children will be signed in and out using the Bright Wheel app provided to parents. LPA's discussed with applicant that a hard copy would be required if the system was down.

LPAs continued to tour the facility and measured all indoor and outdoor activity space. Total indoor activity space measured for the preschool component is 1,276.97 sq ft, which is sufficient to accommodate the requested overall capacity. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, cots, bookshelves, and other activity supplies for the children. Drinking water will be available in the classrooms via the children's brought water bottles. The facility has Brita filtered water and disposable cups. LPA's observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. The fire clearance was granted on 08/27/2024.

LPAs observed a total of 3 sinks and 3 toilets available for children’s use. These are sufficient to accommodate the requested overall capacity. There is a separate staff restroom equipped with a toilet and a sink in the staff lounge next to the kitchen. The isolation area for children who are ill will be in the lobby area near the directors desk.



The purple room was previously set up to receive infant children, however, Ms.Gates will no longer use the room, which has been listed as off limits on the facility sketch (floor plan). Ms. Gates understands that children may not be in those rooms at any time. Ms. Gates also understands that they must maintain enrollment limited to current space available.

Facility will provide morning snack and afternoon snack. The parents/caregivers will provide lunch. The kitchen area currently includes refrigerator, freezer, one microwave, sink and an air fryer. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Food will be observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children upper cabinets in the kitchen.

The facility currently has a fully fenced playground area for the preschool component. Wrought Iron fencing surrounding the property is at least four feet high. The total square footage for all the outdoor activity space for the preschool component is 1,304.19 sq ft, which is not sufficient to accommodate the requested capacity. Shade is provided via gazebo, and side of the building. There are sufficient outdoor age-appropriate toys and play equipment available on the playgrounds. There is a climbing structure on the playground which is age appropriate for the groups that will use them. The preschool playground manufacture recommendation is for 2 years to 5 years old. All equipment is properly anchored. There is not adequate cushioning in fall zones of climbers provided.(see correction needed) Drinking water is available outdoors via the childrens water bottles. LPA's observed all hazardous items on the playgrounds to be inaccessible to children. The applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).



The applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections were noted, the applicant will submit pictures identifying the corrections.
Orange Preschool Room:
1. Anchor Kitchen Play set and cabinet.
2. Anchor Teacher desk.
3. Repair chipped paint on all walls.
4. Repair exposed dry wall on all walls.

Green Preschool Room:
1. Repair chipped paint on all walls.
2. Repair dry wall holes on all walls.

Blue Preschool Room:
1. Repair of chipped paint on all walls.

Preschool Restroom:
1. Removal of doors in each stall (3).
2. Repair the caulking on the left hand side hand washing sink.
3. Repair or replace broken base board behind toilet.
4. Repair the cracks along wall of sinks.
5. Proof that the needed 3rd sink has been installed, shall be submitted.

Preschool Playground:
1. Repair or replace bent wrought iron gate near the climbing structure.
2. Repair the concrete near the canopy.
3. Place adequate cushioning on the fall zones of the climbing structure.
4. Proof that the smaller broken activity structure is removed.

A license for 36 preschool children will be granted upon a final file review and the approved granted waiver for the playground.

LPA will obtain an updated Fire Clearance to reflect preschool only.

Exit interview conducted and report was reviewed with the Ms. Gates.
