On the date and time listed above, Licensing Program Analyst (LPA) Giselle Carbullido conducted a subsequent complaint investigation to deliver final findings. Initial visits were conducted on 02/05/26 and 2/19/26, at which time LPA conducted interviews and reviewed records. LPA met with facility representative, Leslie Torres; toured the facility and took a census.
During the investigation, LPA reviewed records and interviewed pertinent parties, including facility staff. LPA was unable to obtain supporting information pertaining to dates or from all individuals involved in this allegation.
It was alleged staff made inappropriate comments to a child by stating they were going to wash out the child’s mouth with soap. All staff denied making an inappropriate comment and reported the comment may have come from an authorize representative.
It was alleged that staff threatened a child by saying, “Don’t make me call mom and tell her to get the

Due to conflicting information obtained during interviews conducted from what was alleged, the evidence collected was not sufficient to substantiate or refute the above allegations. Although the allegations may have happened, or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
An exit interview was conducted, a copy of this report and Notice of Site Visit was provided to the facility representative, Leslie Torres.
LPA observed the Notice of Site Visit was posted by staff. THIS REPORT MUST BE AVAILABLE TO THE PUBLIC FOR THREE YEARS
