25 | On 10/24/2024, at approximately 12:20 PM, Licensing Program Analyst (LPA) Jesse Gardner was inside the facility conducting an investigation. During a review of records, Staff One (S1) was found to be employed since 04/23/2023, and during which, had not initiated a background check through the Department of Justice (DOJ). S1 doesn't work directly with children; however, has an office inside the facility and works inside that office Monday - Friday 09:30 AM - 6:00 PM which is around children in care and often meets with parents.
Based on interviews and record review, the facility is being cited for Title 22, division 12, chapter 1, article 06; section; 101170(e)(1) Criminal Record Clearance.
A Civil Penalty of $500.00 was assessed for this violation of Title 22 Regulations. See LIC421BG.
An exit interview was conducted. A copy of this report, LIC809D, LIC421BG and appeal rights were reviewed and handed to the Director.
The Director was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door.
If the facility receives a Type A violation, the licensee shall post and provide copies of the report to parents/guardians of the children in care at the facility by the next business day and shall provide to the parents/guardians of children newly enrolled at the facility during the next 12 months. In addition, the licensee shall immediately post upon receipt the Proof of Correction for 30 consecutive days and provide a copy to current and enrolling parents. The licensee is to keep Acknowledgement Receipt (LIC 9224) signed by parents in each child’s file. This is an amended copy of the original report dated 10/24/2024. |
