On Wednesday, November 6, 2024 at approximately 08:39 AM, Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on information received by the Department. LPA met with Director Mari Rivera (S1) and later toured the facility.

It was reported by confidential witnesses that the facility had not provided information to parents related to the Type A citation received by the facility on 10/24/2024.

S1 stated that they had sent notice to parents regarding the outcome of LPA's visit on 10/24/24; however, the notice did not include the actual report left by LPA. Deficiency was cited.

On today's visit, 11/6/2024, LPA observed that the report (that was left on 10/24/24) was not posted with the other postings for parents to review. A deficiency and an immediate $100 civil penalty was issued as a result.

An exit interview was held with Director Mari Rivera and a copy of this report, along with copies of the LIC809D (deficiecy page), LIC421CC (Civil Penalty page), and Appeal Rights were discussed and provided. A Notice of Site visit was issued and must remain posted for 30 days. This report shall be public record for three years.

Posting licensing report by child care facility or home..: (a)(1) Each licensed child day care facility shall post a copy of any licensing report pertaining to the facility that documents either a facility visit or a complaint investigation that results in a citation for a violation that, if not corrected, will create a direct and immediate risk to the health, safety, or personal rights of
children in care. The licensing report provided by the department shall be posted immediately upon receipt, adjacent to the postings required. This requirement was not met as evidenced by:
